APPLICATION IS ATTACHED BELOW WITH ADDITIONAL INSTRUCTIONS. The City of Phoenix created the Office of Accountability and Transparency (OAT) to perform independent civilian oversight of the Phoenix Police Department. OAT reviews Department administrative investigations of critical incidents involving sworn personnel and provides community members a way to freely communicate complaints, commendations, and concerns about police officers and the Phoenix Police Department without fear of retaliation pursuant to Chapter 20 of the Phoenix City Code. The Civilian Review Board shall review, discuss and approve monitoring reports issued by the Office of Accountability and Transparency; review and discuss OAT policy recommendations to the Phoenix Police Department; and review complaints and ensure notification to complainants of the status and disposition of their complaints against Phoenix police officers. The Board shall review and discuss completed OAT monitoring reports, the Phoenix Police Department’s responses to those reports, and make one of the following findings with respect to OAT’s recommendations and determinations of thoroughness and completeness: Adopt, Adopt with Revisions, Decline, Decline with Revisions, or Request Additional Information.

The City of Phoenix Civilian Review Board will consist of nine voting members nominated by the Mayor and approved by an affirmative vote of two-thirds of the members of the City Council for three-year staggered terms. Members will serve no more than two terms. All voting members shall either reside or work in the City of Phoenix and must be at least 21 years of age. No current or former sworn law enforcement officer is eligible to serve as a voting member of the Board. No individual who has a pending claim or lawsuit or who within the 10 years preceding appointment has filed a claim or lawsuit related to actions by the Phoenix Police Department as stated in Phoenix City Code Section 20-6 - Mandatory Oversight by OAT - is eligible to serve as a voting member of the Board.
